All potential residents of Alexandra House have to be referred via the Continuing Support Teams. These assessments can be carried out at home or hospital. Potential residents will be invited to Alexandra House on several occasions and will be required to stay overnight, whenever possible, before being accepted and before a decision is made to move in.
As part of the assessment procedure, residents have staff support for the first 4 weeks for activities such as shopping and cleaning. After this trial period the resident and staff jointly assess the appropriate level of continued support.
Residents complete a self-assessment form prior to moving in. This identifies areas to be included in the care plan, drawn up between the resident and staff, and will include daily living skills such as cooking, budgeting and managing medication. The in-house key worker will review the care plan with the resident on a weekly basis.
