You require housing support. (You are homeless or your home is unsuitable)
You are supported by a member of West Norfolk CMHT.
(We require a full CPA and risk assessment from the CMHT, you will need to fill in a self-assessment form so we can see what support you require).
How we will support you to achieve your goals.
We support you by looking at all areas of your life with you:-
- What you used to do that you enjoyed in your life.
- What you do now.
- What do you wish to bring back into your life.
- What new goals do you wish to achieve and how we can support you in achieving them.
- We will encourage you to maintain your usual activities and to add additional activities to them.
